I have MIT Medical / Harvard University Health Services. Should I go through them instead?
Often, yes — if you can get on the calendar. Both are excellent primary-care systems, and if your affiliation covers testosterone therapy under a covered indication, that path is usually cheaper than Aurafil out-of-pocket. The tradeoffs: (1) wait times for endocrinology consultation; (2) whether you want a specific treatment recorded in your primary employer's health record. I'm a Harvard postdoc — will my J-1 visa or immigration status be affected?

My testosterone from a Kendall Square biotech company wellness screening was 380 ng/dL. Am I hypogonadal?
Probably not by the Endocrine Society definition (which uses ~264-300 ng/dL as the lower bound), but symptoms in the presence of a low-normal number deserve a proper workup — a second morning draw, free testosterone with SHBG, LH/FSH, prolactin, and a full differential (sleep apnea, thyroid, iron, mood). Can I get my testosterone shipped, or do I have to pick it up?
Both options work. Aurafil's default is home shipping from a MA-licensed compounding or dispensing pharmacy — cold-chain packaged, tracked, typically 3–5 business days. If you prefer local pickup, we can route the prescription to a MA-licensed retail pharmacy in Cambridge (CVS or Walgreens in Harvard Square, Central Square, or Porter). Discuss preference at your video visit.
I have a family history of prostate cancer. Can I still get evaluated?
Yes, but the workup and monitoring plan are more careful. The 2018 Endocrine Society guideline recommends against testosterone therapy in men with active prostate cancer and calls for individualized risk-benefit discussion in men with a strong family history or elevated PSA. At intake we screen for these factors and order baseline PSA if age-appropriate; red flags trigger a urology referral before initiating therapy.
What if my labs come back normal?
Then we don't prescribe. Prescribing testosterone to a man with normal levels is neither Endocrine Society-compliant nor good medicine. What we do instead is help you understand the differential and refer to the appropriate specialist — sleep medicine, endocrinology, primary care — depending on what the workup suggests. Our mission is compliant, honest care, not maximum prescriptions.
