I have a family history of prostate cancer. Can I still get evaluated?
Yes, but the workup and monitoring plan are more careful. The Endocrine Society's 2018 guideline recommends against testosterone therapy in men with active prostate cancer and calls for individualized risk-benefit discussion in men with a strong family history or elevated PSA. At intake, we screen for these factors, order a baseline PSA if age-appropriate, and — if there are red flags — refer you to a urologist for evaluation before initiating therapy. Testosterone is not appropriate for every man, and a compliant clinic will tell you so.
What if my testosterone comes back normal?
Then we don't prescribe. Prescribing testosterone to a man with normal levels is not Endocrine Society-compliant and is not good medicine. What we do instead is help you understand the differential — sleep apnea (very common in shift-workers), thyroid dysfunction, iron deficiency, depression, alcohol use — and refer you to the appropriate specialist. Aurafil's mission is compliant care, not maximum prescriptions.
